To receive Optical Benefits (eye exam and glasses), you need an Optical Voucher to present to the Optical provider. You can request a Voucher by completing the form below and clicking on "Submit Optical Form." The Fund Office will process your request and mail you the voucher(s). Please indicate for whom you need the voucher(s) - yourself, spouse or child.
